When you fill out a facility request form you need to put Swoosh/your team name as the renter. Each additional site certificate is $25.00. Keep in mind a site certificate that names the school district covers all the schools in that school district. You do not need to buy additional certificates.

Liability Insurance for up to ten players/coaches (does not include medical)
